You must own a vessel or listed as alternate skipper on a vessel.

Each request for an application has to be given individual consideration.
There are two types of applications available. One for vessel owners, and one for all other members, i.e., co-owners and alternate skippers.
Each application package consists of: 1) Application form as applicable, 2) Waiver of Salvage Rights, 3) Acknowledgment and Waiver.
A copy of the valid CSI Certificate must accompany all applications from vessels 15 GRT and over.
Proof of vessel ownership is required, i.e., certificate of registry, or some other document which indicates vessel ownership.
Once a request is received for an application package, it has to be approved by the appropriate District Director before he/she is sent the applicable information.
